Stedman Community Hospice is a leader in end-of life care, where the focus remains on living, not dying.
Adjacent to the main building of St. Joseph’s Lifecare Centre in Brantford, the 7,200 sq. ft. Hospice is a free-standing home that offers a variety of programs and services to meet the needs of the terminally ill and their families.
So much has changed since Stedman Community Hospice opened our doors and we have far surpassed our initial hopes and expectations in providing compassionate care for the terminally ill patients and their families. We have initiated a number of “provincial firsts”, putting Brantford on the map as a model of excellence in hospice care. In 2008, one of those "firsts" was the Community Outreach Program. This program sends an interdisciplinary team of professionals, including a doctor, to the homes of terminally ill patients in crisis, to provide pain and symptom management and thus alleviate the need for emergency room visits. Last year alone, we saved 195 hospital admissions translating to a conservative estimate of $2.3 million in funding that can be utilized to treat the right person, in the right bed, at the right time. When the program began they were visiting an average of 30 patients per month, last month that number reached 314 patients!
The Community Outreach Program has impacted all of the programs and services provided at Stedman Community Hospice. The Hospice offers both Residential In-patient Programs as well as extensive Day Wellness programming. Our Residential Program demands have increased more than 260% and we have welcomed more than 4,500 visits through our Day Wellness Program.
The largest increases have been through our Bereavement and Grief Support Programs where we have seen more than an 1,100% increase in just five years. We find for every Hospice patient we help, we provide bereavement care for FIVE additional family members.
All of the Hospice programs and services are community funded and therefore anyone, at any age or stage of a life threatening illness can count on the Hospice without having any financial implications and the Hike for Hospice provides a major source of this funding. All funds raised at the Hike in Brantford, stay in Brantford to help Stedman Community Hospice.
We are blessed to have more than 100 dedicated Volunteers, who contribute over a thousand hours monthly, to help with the tasks and activities taking place at the Hospice and ensure that we can operate our programs at the lowest possible cost. The wonderful, compassionate, caring work that is being done by staff and volunteers is never-ending.
